Individual Service Plans at Missing Links

Individual Service Plans itemize specific goals for the services each child receives, guide the program and link the child and family to appropriate community services.

Each Missing Links Individual Service Plan:

  • outlines the services the child requires
  • identifies attainable goals for the child
  • guides his/her Intensive Behaviour Intervention (IBI) while in the program
  •  indicates the level of intensity, the setting, and the duration of the IBI
  • links the child and family to appropriate and available community services
  • sets out the roles of the family
  • provides transition planning for the child’s next environment
  • is reviewed and modified at regular intervals
  • is signed by the parents, and if appropriate, by other community service providers involved in the plan’s implementation
